Eli Lilly and Company launched the Lilly Change the Course Commitment on Monday, Sept. 28, 2026, according to a report by PR Newswire M&A (source). The global health initiative aims to achieve patient parity by treating as many people with diabetes and obesity in resource-limited settings, including low- and middle-income countries, as in high-income countries before 2040. Currently, for every 4 people in high-income countries who access these medications, only 1 person in low- and middle-income countries can reach them. Four in five adults with diabetes and 7 in 10 adults with obesity live in LMICs, where prevalence continues to rise and patients have historically waited decades for innovative therapies.

Three Pillars of the Initiative

To close the access gap, the initiative focuses on three core pillars. First, Lilly is working to ensure diabetes and obesity medicines are suitable for resource-limited settings from the earliest stages of clinical development, accounting for clinical trials, medical presentations, and cold chain storage. Second, the company intends to register innovative treatments, including GLP-1s, in more low- and middle-income countries, prioritizing Africa, Latin America, and Asia. Third, Lilly plans to collaborate with global health organizations to strengthen primary health systems by training community health workers, leveraging technology and digital tools, and improving disease awareness, prevention, diagnosis, and care.

Preceding Success and Leadership Perspective

The new commitment follows the completion of the Lilly 30x30 global health initiative, which expanded access to quality healthcare for 40 million people in resource-limited settings in 2025. That prior effort surpassed its original goal of helping 30 million people annually by 2030, finishing 5 years ahead of schedule. David A. Ricks, Lilly chair and chief executive officer, stated that innovation only matters when it reaches the people who need it. As part of this expansion, products such as Foundayo, an oral treatment that can be stored at room temperature, will help distribute and scale access across more LMICs. Orforglipron was originally discovered by Chugai Pharmaceutical Co., Ltd. and licensed by Lilly in 2018.

Foundayo Safety and Regulatory Details

Foundayo is FDA-approved for adults with obesity, or certain adults with overweight who have weight-related medical problems, to reduce excess body weight and maintain weight reduction alongside a reduced-calorie diet and increased physical activity. The drug carries safety warnings regarding thyroid tumors, including medullary thyroid carcinoma, and should not be used by patients with Multiple Endocrine Neoplasia syndrome type 2. Other serious side effects include pancreatitis, severe stomach problems, dehydration leading to kidney problems, low blood sugar when combined with insulin or sulfonylureas, serious allergic reactions, vision changes, gallbladder problems, and aspiration risks during anesthesia.
